An episiotomy ( /ɛˌpiːziːˈɒtəmiː/), also known as perineotomy is a surgically planned incision on the perinium and the posterior vaginal wall during second stage of labour. The incision, which can be midline or at an angle from the posterior end of the vulva, is performed under local anaesthetic (pudendal anesthesia), and is sutured closed after delivery. It is one of the most common medical procedures performed on women, and although its routine use in childbirth has steadily declined in recent decades, it is still widely practiced in many parts of the world including Latin America, Poland, Bulgaria, India and Taiwan.

Όταν το κεφάλι του εμβρύου πιέσει το περίνεο, κάνοντάς το πολύ λεπτό, τότε ο γιατρός κάνει περινεοτομή. Δηλαδή μια τομή στην περιοχή ανάμεσα στον κόλπο και τον πρωκτό.
Η περινεοτομή βοηθάει την έξοδο του μωρού αλλά προλαμβάνει και από τυχόν μεγάλο ή ανεξέλεγκτο σχίσιμο του περινέου.
Μετά την έξοδο και του πλακούντα ο γιατρός συρράπτει την περινεοτομή με ειδικά ράμματα, τα οποία θα απορροφηθούν και δε χρειάζεται να κοπούν.
